File Upload Requirement
Acceptable file formats include PDF, Word documents (.doc, .docx), and text files. Ensure your documentation is complete and reflects all changes made to the program.
Types of Substantive Changes
The following modifications are considered substantive changes that require documentation review:
Curriculum and Content Changes:
- Addition, removal, or significant modification of course content
- Changes to learning objectives or outcomes
- Alteration of program sequence or prerequisites
- Modification of clinical or practical training requirements
Structural and Administrative Changes:
- Changes in program duration or contact hours
- Modification of faculty qualifications or teaching assignments
